Job Introduction

Nurse Assessor - Office hours at home then travel for face to face Assessments 

Salary:  £40,000 - £45,000

Job Purpose

Reporting to the Head of Nursing, you will lead on clinical assessments ensuring that we are able to deliver individualised support packages. You will work closely with the Assessment and Contracts Manager to ensure the packages are deliverable and in line with our model of support across an agreed geographical area.


Main Responsibilities:

  • Completing desk-based and face to face assessments of referred people for support. 
  • Ensuring an APIE approach - Assessing, planning, implementing and evaluating. 
  • To review reports and evidence outcome measures to inform clinical practice, business performance and quality measures.  To maintain continuous professional development and be able to produce your CPD folder upon request. 
  • Able to identify and mitigate complex risks for a community setting. 
  • To review pre-admission assessment in a timely and professional manner. 
  • Producing clinically informed assessments with robust risk assessment and identifying risk mitigation measures. 
  • Identify measures to ensure people can be enable through positive support. 
  • Attend internal and external MDT meetings to further informed assessment and risk mitigation. 
  • Building professional working relationships. 
  • Effective time management. 
  • Record keeping, ensuring that accurate safe and contemporaneous records are maintained. To effectively work alongside stakeholders and other external professionals
  • Complete assessments based on current information, that are evidence based and which identify risk mitigation.
  • Identify which completed assessments can be progressed to Business Development for presentation to Commissioners as they fit within our model of support and can be safely supported by Gray Healthcare.


To be considered for the role you must closely match the following criteria:

  • Aegistered Mental Health Nurse, Occupational Therapist - ESSENTIAL
  • Experience of assessing and working with people with a variety of complex needs around their mental health, learning disability, ABI, personality disorder, autism and forensic backgrounds. 
  • Understanding of continuing healthcare or transition of individuals moving from hospital/residential settings into the community and a collaborative, solutions-focussed approach to any issues associated with this 
  • Applied knowledge of the Mental Health Act, CTO’s and the Mental Capacity Act, Deprivation of Liberty Safeguards and Best Interest Decision making 
  • An expert in assessment and management of risk you will be an innovative thinker and natural communicator with strong report writing skills and computer literacy and an ability to work under pressure. 
  • National travel and overnight hotel stays may be required
  • Full UK Driving Licence and access to a vehicle
